Follow a made-up athlete through seven clubs. At the last, in Qatar, no part of your record reaches a doctor by itself. For 19% of the pieces along the way, no source says either way. Fly from the United States to France and Italy, and no part of your record reaches a doctor there by itself. You can bring a copy of all 5 parts.
At each stop on a route the figure asks how many of the five parts of the record held at home (patient summary, prescriptions, lab results, images, clinical notes) reach the doctor there, and by which channel, against what the person has at home and what EU law promises by 2031. A visitor's record is pulled from home at every stop; it does not hop from stop to stop. When you move, the country you leave keeps part of your record. Five parts, fixed lanes of equal width: a missing image matters differently from a missing summary, and the count counts parts without weighing them. No portability score is computed.
A border is read from both sides. The home country (or each earlier home) must send the part and the stop must receive it. If both run the service it is live. If either side has nothing, it is none found, and a known break outranks a gap in knowledge. If either side is not known, it is not known. If either side is only scheduled in law, it is scheduled, with the later of the two years. Inside the EU a part that does not cross today is scheduled with its EHDS year (2029 for summaries and prescriptions, 2031 for the rest).
Where national sources and the Commission's transaction data disagree (Hungary (prescriptions, both ways), Lithuania (patient summaries, both ways) and Poland (patient summaries, receiving)), the side is drawn as not known rather than picking one.

Where a preset needs a neighbour, it takes the nearest rated country sharing a land border with home, or the nearest by distance; a free route takes up to 8 stops, and a country comes back only as a return to home or to a place the person lived.
The year beside each lane name is the EU law date for that part (EHDS, Article 105): a promise in law, not practice, and it binds only pairs where both sides are EU members. Under a stop with no such pair the figure prints "EHDS does not apply"; under Norway it prints "EHDS not verified (EEA)", because whether the EHDS dates bind Norway, Iceland and Liechtenstein was not checked. "Not known" and "live if visiting" under a moved stop count holder cells.
The locator draws the route in ink with stops numbered in order; distance is not data, order is. In the home column, the seven markers show how connected the record is at home, a different measure from crossing a border (filled connected, half partial, open siloed, dashed not known). Across all 198 country files, 345 of 1,386 home journey stages (25%) are not known.
We chose the sample routes to show different outcomes, so they are not typical. Note 2 gives the EU-wide count.
